Providing Proof of Vaccination Status
As part of our ongoing efforts to keep everyone healthy, safe and in school, we are strongly encouraging, but not requiring, our students and staff to get the COVID-19 vaccine and booster. We are also encouraging those who get vaccinated to provide us with proof of their vaccination status. Having access to this information will enable us to adjust or modify our plans when updates are made to the guidance for schools or there are changes in the transmission level for the community. It will also help us streamline the contact tracing process and make it easier to identify those who will need to quarantine should we experience a positive case.
If you would like to provide us with your child’s vaccination record, you can bring a copy of it to their school or use the information below to email it or fax it to our nurses.

* Please note, if you are using an iPhone to send a picture of your child’s vaccination card, please change the image format from HEIC (High Efficiency) to Most Compatible. The majority of the devices we use in our district are PCs, which cannot read the HEIC format.
Monitoring District 86 Vaccination Rates
We continue to monitor the student and staff vaccination rates for the district, and are sharing that information with the community on a weekly basis. You can access the most up-to-date rates, as well as the data for positive cases, people in quarantine and people in isolation, by visiting our COVID-19 dashboard.
